Enrollment and Withdrawals

Enrollment and Withdrawal forms can be completed ahead of time.  The legal parent/guardian must make an appointment to come into the Registrar's Office to finalize the process.

The Registrar's Office is open weekdays (except for holidays).

Enrollment:


A custodial parent or legal guardian must enroll the student.  The Enrollment Form can be completed ahead of time and all pertinent documents can be mailed / faxed / emailed or brought in to our office.  The legal parent or guardian must make an appointment to finalize the enrollment process in person (a government issued picture ID will be required).  Students will not be able to attend classes until the enrollment process is complete.

  1. If your student is coming from a Hawai'i Public School, please bring with you:

    • Release packet from releasing school

      • Certificate of Release

      • Copy of health record (Form 14)

      • Pupil clearance or transfer form, as applicable

      • Current Report Card or withdrawal/transfer grades

      • Official Transcript

      • Copy of SpEd current IEP or 504 records, as applicable

    • 2 Proofs of residence address *School Site Locator (Click to see school district lines and/or which school district a residential address is within - ALWAYS contact the school to verify that the address is within their district as the site is not always up to date.)

      • If renting:  Rental/Lease Agreement and 1 current utility bill

        • Must have parent/guardian's name

        • Must have physical address

      • If own:  2 current utility bills, property tax or escrow documents

        • Must have parent/guardian's name

        • Must have physical address

      • If living with another family​

  2. If your student is coming from a Hawai'i Private School, please bring with you:

    • All items listed for  #1 (from a Hawai'i Public School)

    • Birth Certificate

  3. If your student is coming from Another State or a Foreign Country, please bring with you:

    • All items listed for #1 (from a Hawai'i Public School)

    • Birth Certificate

    • TB clearance (within 12 months, performed and signed by a US physician/medial facility, APRN, PA or DOH)* For information regarding TB clearance requirements, talk to your child's healthcare provider or call the DOH Tuberculosis Control Branch at (808) 832-5731 or visit health.hawaii.gov/tb.

    • Physical Exam (within 12 months, performed and signed by a US physician/medial facility, APRN, PA or DOH)* The appropriate sections of Form 14 can be completed to document this.

  4. Geographic Exception form, if applicable.

Withdrawal:

A custodial parent or legal guardian must withdraw the student.  The Withdrawal Form can be completed ahead of time and can be mailed / faxed / emailed or brought in to us.  The legal parent or guardian must make an appointment to finalize the withdrawal process in person (a government issued picture ID will be required). 

Parents/Guardians should enroll their student at the new school within:

  • a few days if enrolling on Hawai'i island

  • one (1) week if enrolling on another Hawai'i island

  • two (2) weeks if enrolling out of state

Students will not be released from Waiakea High School until the withdrawal process is complete and we receive a notice of enrollment from the receiving school.

Keep in mind that all absences accumulated due to moving schools is subject to the State of Hawaii's compulsory attendance law (Hawaii Revised Statutes 302A-1132). can be viewed here.  Absences due to moving that become chronic absentees may be referred to Family Court. Pursuant to H.R.S. 571-11(2), Family Court can place a child under jurisdiction for truancy if the child is not attending school or is not receiving the educational services required by law.
